2025 SEAHAWKS MENTAL HEALTH MATTERS

WELLNESS ROOM GRANT  

1. Eligibility & Guidelines: To apply for a 2025 Seahawks Mental Health Matters Wellness Room Grant (the "Grant"), your organization must meet all the following eligibility criteria and guidelines:

  • Organization must provide programs or services in one of the following states: Washington, Oregon, Alaska, Montana, Hawaii, or Idaho.
  • Grant considerations will be given to organizations with community projects and programs that align with the pillars of the Seattle Seahawks community values. Grants will not be awarded to support administration or operating costs, fundraising efforts (including special event tables, event sponsorships, or auction items), religious organizations, individuals, endowments or reserve funds, annual memberships, political campaigns, conferences, workshops, group or individual travel, or individuals fundraising on behalf of a charity.
  • Each organization may submit only one (1) application during the Submission Period (as defined below).
  • The application must be submitted by an individual 18 years of age or older.
  • Submission of an application does not guarantee that your organization will receive a Grant.
  • Due to the volume of applications received, the Seattle Seahawks are unable to provide feedback on applications that are not selected. All communications will be limited to the organization(s) chosen to receive a Grant.

Directors, officers, members, managers, and employees of Football Northwest LLC d/b/a Seattle Seahawks ("FNW"), the National Football League ("NFL") and its member clubs, and each of their respective parent companies, affiliate companies, subsidiaries, agents, professional advisors, advertising, promotional, public relations, and fulfillment agencies, consultants, legal counsel, website providers, web masters, and immediate family members (spouses, parent, child, sibling, grandparent, and "step" child, wherever they may live) of each are not eligible to apply for the Grant.  IF YOU DO NOT MEET THESE REQUIREMENTS, YOU ARE NOT ELIGIBLE TO APPLY FOR A GRANT.  To be eligible to be selected for a Grant, your application must be completed and received by FNW in the manner and format designated below. 

2. Submission Period:  Applications must be submitted between 12:00am Pacific Time (PT) on December 12, 2025 and 11:59pm PT on January 12, 2026 (the "Submission Period").

3. How to Apply: To apply, (i) visit Wellness Room Grant Application Form and (ii) complete and submit the 2025 Seahawks Mental Health Matters Wellness Room Grant application form.

By applying as instructed above, you agree that your application does not supply untruthful, incomplete, inaccurate, or misleading information

If applying via your mobile device (which service may only be available via select devices and participating wireless carriers and is not required to enter), you may be charged for standard data use from your mobile device according to the terms in your wireless service provider's data plan.  Normal airtime and carrier charges and other charges may apply to data use and will be billed on your wireless device bill or deducted from your pre-paid balance.  Wireless carrier rates vary, so you should contact your wireless carrier for information on your specific data plan.

4. Selection and Notification of Grant Recipient(s): On or around February 1, 2026, a panel of judges selected at FNW's discretion will score each application based on the considerations outlined in Section 1 above.

Up to five (5) organizations will be deemed the selected Grant recipients (each a "Grant Recipient" and collectively, the "Grant Recipients").

Grant Recipients will be notified via email by February 13, 2026. To the fullest extent permitted by applicable law, FNW may require the Grant Recipients to sign and return, as specified by FNW, an affidavit to claim the Grant.  If (i) the attempted notification is returned as undeliverable without a forwarding address or a selected Grant Recipient does not respond within one (1) week of being notified by FNW; (ii) any required documents are not returned as specified by FNW; or (iii) if the selected Grant Recipient is otherwise unable to accept the Grant, the Grant will be forfeited and will be awarded to the individual with the next top scoring application.

Verification of Grant Recipient: EACH SELECTED GRANT RECIPIENT IS SUBJECT TO VERIFICATION BY FNW, WHOSE DECISIONS ARE FINAL AND BINDING IN ALL MATTERS RELATED TO THE GRANT.  AN ORGANIZATION IS NOT A GRANT RECIPIENT UNLESS AND UNTIL ORGANIZATION'S ELIGIBILITY HAS BEEN VERIFIED AND ORGANIZATION HAS BEEN NOTIFIED THAT VERIFICATION IS COMPLETE AND NO FURTHER ACTION IS NEEDED.

5. Grant: Each Grant Recipient will be awarded between $2,500 and $5,000 to support the creation of a mental health wellness room. Eligible expenses include any costs that assist with the design, construction, enhancement, or activation of a mental health wellness room. Allowable uses of funds may include, but are not limited to, supplies, furnishings, programming or event materials, contractor or labor fees, and other project-related expenses.

Award Restrictions: ALL FEDERAL, STATE AND LOCAL TAXES ASSOCIATED WITH THE RECEIPT OR USE OF ANY GRANT ARE THE SOLE RESPONSIBILITY OF THE SELECTED GRANT RECIPIENT.

6. Disclaimer:  FNW and any of its respective parent companies, subsidiaries, affiliates, directors, officers, professional advisors, consultants, contractors, legal counsel, public relations firms, employees and advertising, fulfillment and marketing agencies (collectively, the "Released Parties") will not be responsible for: (a) any late, lost, misrouted, garbled or distorted or damaged submissions; (b) phone, electronic, hardware, software, network, Internet, or other computer or communications-related malfunctions or failures beyond FNW or the Released Parties' reasonable control; (c) any Grant disruptions, injuries, losses or damages caused by events beyond the reasonable control of Sponsor or the Released Parties by non-authorized human intervention; or (d) any printing or typographical errors in any materials associated with the Grant.

7. Limitations of Liability; Releases:  By applying for a Grant, you release Sponsor and all Released Parties from any liability whatsoever, and waive any and all causes of action, related to any claims, costs, injuries, losses, or damages of any kind arising out of or in connection with the Grant or delivery, misdelivery, acceptance, possession, use of or inability to use any Grant (including claims, costs, injuries, losses and damages related to rights of publicity or privacy, defamation or portrayal in a false light, whether intentional or unintentional), whether under a theory of contract, tort (including negligence), warranty or other theory. 

8. Use of Grant Recipient's Name, Logo(s), etc.: Application for a Grant constitutes permission for FNW and the NFL to use the name, logo(s), and other intellectual property of your organization for lawful purposes in any media, worldwide, in perpetuity, without further notice or compensation, except where prohibited by law. As a condition of being awarded any Grant, except where prohibited by law, Grant Recipients may be required to execute a consent to such use. By applying for a Grant, you consent to being contacted by FNW for any purpose in connection with this Grant.  

9. License to Submission: By applying for this Grant, you hereby grant to FNW a perpetual, irrevocable, royalty-free, worldwide, nonexclusive license to publish, reproduce, display, perform, distribute, adapt, edit, modify, translate, create derivative works based upon, and otherwise use and sublicense your submission, or any portion thereof, in connection with the Grant and for other advertising, marketing, and promotional purposes, and to incorporate submissions, in whole or in part, into other works in any manner, form, media or technology now known or later developed. FNW will have no obligation to publish or use or retain any application you submit or to return any such application to you.  You agree that it is your sole responsibility to obtain all permissions and releases necessary for the grant of the rights contained in this paragraph. You agree to take, at FNW's expense, any further action (including execution of affidavits, tax forms, and other documents) reasonably requested by FNW to effect, perfect or confirm FNW's rights as set forth above in this paragraph.  You will not be entitled to compensation for any use by FNW, or its agents, licensees or assignees, of your submission.

10.  Privacy:  Any information you provide in connection with the Grant will be processed in accordance with FNW's Privacy Policy, which is accessible at http://www.seahawks.com/about/privacy-policy.

11. Intellectual Property Notice: All Seahawks-related trademarks are trademarks of Football Northwest LLC d/b/a Seattle Seahawks ("FNW").  The Grant Eligibility & Submission Requirements and all accompanying materials are copyright © 2025 by FNW.  All rights reserved.

12. Disputes:  EACH APPLICANT AGREES THAT ANY DISPUTES, CLAIMS, AND CAUSES OF ACTION ARISING OUT OF OR CONNECTED WITH THIS GRANT WILL BE RESOLVED INDIVIDUALLY, WITHOUT RESORT TO ANY FORM OF CLASS ACTION, AND EXCLUSIVELY BY THE UNITED STATES DISTRICT COURT FOR THE WESTERN DISTRICT OF WASHINGTON OR THE APPROPRIATE STATE COURT LOCATED IN KING COUNTY, WASHINGTON. THESE ELIGIBILITY & SUBMISSION REQUIREMENTS ARE GOVERNED BY THE LAWS OF THE STATE OF WASHINGTON, WITHOUT REGARD TO CHOICE OF LAW OR CONFLICT OF LAW RULES.  YOU WAIVE ANY AND ALL OBJECTIONS TO JURISDICTION AND VENUE IN THESE COURTS AND HEREBY SUBMIT TO THE JURISDICTION OF THOSE COURTS. 

13. General Conditions: Applicants agree to be bound by these Eligibility & Submission Requirements and by the decisions of FNW, which are final and binding in all respects.  FNW reserves the right to change these Eligibility & Submission Requirements at any time, in its discretion, and to suspend or cancel the Grant or any applicant's participation in the Grant should viruses, bugs, unauthorized human intervention or other causes beyond FNW's reasonable control affect the administration or security of the Grant, or FNW otherwise becomes (in its discretion) incapable of granting a Grant as planned.  Applicants who violate these Eligibility & Submission Requirements, violate any law, rule or regulation in connection with participation in the Grant, tamper with the operation of the Grant, or engage in any conduct that is detrimental or unfair to FNW, the Grant or any other applicant (as determined in FNW's discretion) are subject to disqualification from the Grant.  Any attempt by an applicant or other individual to deliberately damage any website associated with this Grant or undermine the legitimate operation of the Grant may be in violation of criminal and civil law and FNW reserves the right to prosecute and seek damages from any such person to the fullest extent permitted by applicable law.  FNW reserves the right to lock out persons whose eligibility is in question or who have been disqualified or are otherwise ineligible to apply for a Grant.  FNW reserves the right to disqualify any applications received that contain inappropriate or offensive content.  FNW's failure to enforce any term of these Eligibility & Submission Requirements will not constitute a waiver of that provision.  Any provision of these Eligibility & Submission Requirements deemed unenforceable will be enforced to the extent permissible, and the remainder of these Eligibility & Submission Requirements will remain in effect.  The use of the terms "include" or "including" in these Eligibility & Submission Requirements is illustrative and not limiting.
